The Center at Arrowhead is a 75,700 square-foot state-of-the-art skilled nursing facility located at 7201 W. Camino San Xavier Ave, Glendale, AZ 85308. They're licensed by the state of Arizona as a skilled nursing facility (SNF) and accept patients who've completed acute hospital care and need structured recovery before going home.

They're not a hospital. They're the step between hospital discharge and home recovery. If you've had surgery, a stroke, a heart attack, or an infection requiring IV antibiotics, or you need complex wound care with frequent monitoring—that's what they handle.

Medically Complex Patients Are Their Specialty: - Oxygen support and frequent monitoring - Breathing tubes, feeding tubes, limited mobility - Large wounds and complex wound care - 24/7 nursing care with routine blood draws, medication delivery, X-rays

Plus: Diabetes management, kidney failure, cancer care, cellulitis, diverticulitis, hypertension, peptic ulcers, and dozens more conditions.

Each patient gets physical therapy, occupational therapy, and speech therapy (if needed):

Home Evaluation Before Discharge: A therapist visits your home several days before you leave to evaluate your space, recommend modifications, and identify equipment you'll need after discharge. They'll order brand-new equipment for you if rehabilitation items prove necessary—walkers, wheelchairs, bedside commodes, etc.

• •Medicare Coverage: Covers skilled care at SNFs under certain conditions. For medically stable patients post-hospital, Medicare covers up to 100 days of skilled nursing and daily skilled services (wound care changes, physical therapy, etc.).

Their case managers coordinate your discharge to home or other facilities (independent living, assisted living, long-term care). They'll order home health therapy and durable medical equipment delivered before you leave.
